A Summary of Our Services:
Private Duty Home Care – Auburn VNA Extended Care provides the care you need at a price you can afford. We provide one-to-one care for clients who may need care all day or intermittently. Private duty services include bathing, dressing, and assistance with personal needs, help walking and with transfers, as well as shopping, cleaning, laundry, companionship and light chores.
Special Services – Auburn VNA Extended Care has developed services that cater to our clients with special needs, including those with life limiting illnesses, dementia, or care for two people in the same home. Caregivers are specially trained to tailor the type and scope of care needed in these unique circumstances. Auburn VNA Extended Care also works closely with its sister corporation, Auburn Visiting Nurse Association and Millbury Adult Day Health, to ensure continuity, communication and sensitivity to our clients’ special needs.
Geriatric Care Management (GCM) – Auburn VNA Extended Care provides professional long-term care planning for people in need of home care assistance. Home care clients may be eligible for local, state or federal home care assistance, and our professional planners provide an assessment and guidance to community program benefits. Through our affiliation with Auburn Visiting Nurse Association, Auburn VNA Extended Care also offers care management and care oversight services from RNs and Therapists. Our GCM program offers the full range of professional guidance, planning, advocacy and care management from your dedicated care manager.
